Participant Form for Evaluating an Event

This form can be done on its own, or after a group conversation so that participants remember details of the event. Written evaluation is especially important if the client expects behavioural problems from participants or if the client expects problems because of the sensitivity of the topic. The fill-in sheet allows participants to comment anonymously. The assessment sheet should contain some questions on process and on content. The results might show that while the content was problematic for participants, they were happy for the chance to participate.
